Primary Care Physicians

As a primary care physician in our organization, you play a central role in the healthcare outcomes of our patients.

Highlights of being a Primary Care Physician with American Care:

  • Responsible for a panel of 350 Medicare Advantage patients.

  • Typically see on average about 14 patients per day

  • Plays a strong role in teaching patients goal-oriented self-care, which improves outcomes.

  • Required adherence to evidence-based medicine that is population targeted.

  • Plays a central role in developing a caring attitude among members of the care team.

  • Accountable for meeting certain performance goals, such as admissions/1000, patient satisfaction levels, completion of care gaps closure, medication refill adherence, and other quality measures.

  • Participates in inpatient care review meetings, daily morning care team planning meetings, training meetings, and other clinical meetings required to improve patient outcomes.

  • Works closely with practice managers and medical directors to achieve patient care goals and company goals.

  • Required adherence to regulatory compliance and the company’s policies and procedures.
